Controlled Substance Policy
At Epiphany Integrated Health, we take a cautious and responsible approach to prescribing controlled substances. While benzodiazepines may be prescribed on a limited, short-term basis, they are rarely used and are not intended for long-term treatment. We are committed to working collaboratively with patients to taper these medications and transition to safer, non-narcotic alternatives over time. Our clinic has a strict no-benzodiazepine policy for individuals also prescribed opioids or other CNS depressants. Additionally, we follow standard care guidelines, which limit the use of short-acting stimulants. Aggressive behavior, non-compliance with appointments, or harassment of staff will result in a safe tapering of medications and immediate discharge from the practice.
Appointments and Cancellation Policy
You are responsible for attending all scheduled appointments and agree to follow the policy outlined below: If you are unable to attend a scheduled appointment, you must notify our office to cancel or reschedule at least 24 hours in advance. The first no-show or late cancellation will be excused. However, a second no-show or late cancellation will incur a $75.00 fee to reschedule. If cancellations or rescheduling occur more than once, we may review your treatment goals and commitment to care.

FAQ's
What fees am I responsible for?
You are financially responsible for the cost of the mental health services provided to you or your dependent(s) and for any portion of the fees not reimbursed or covered by your health insurance, including late cancellations and missed appointments.
Do I need my insurance card?
Yes, please bring your insurance card to each of your appointments. A copy of your up-to-date and active insurance card is required for your appointment. If you do not have a copy of your insurance card, a $50 hold will be placed prior to your appointment. A copy of your insurance card can be uploaded by our staff or through the patient portal.
Do I pay a copay for virtual/telehealth appointments?
Copays are collected prior to your appointment. In-person patients can pay copay at reception, and virtual patients are required to have a credit card on file to bill your copay. If you have insurance, and it is accepted by Epiphany Integrated Health, your insurance will be billed for the appointment. Any copays required by your insurance plan will be billed at time of service. We recommend reviewing your explanation of benefits from your insurance company so that you know what to expect. For patients with no insurance, please inquire with your office for sliding scale and self-pay options.
How do I request prescription refills?
If your medication needs a refill, please ensure your pharmacy does not already have a refill on hold for you, especially if you have seen your provider recently.
If you have not seen your provider in the last 90 days, you will need to schedule a follow up appointment to obtain refills. Please use the “Book Appointment” button on this website to get that scheduled.
If you’ve been seen recently, please let us know which medication you’re requesting, and we’ll get a message over to your provider. Please be aware that controlled substances may or may not be refilled before your next appointment.
